HEDIS Medical Record Collector
Medical Records Collector (HEDIS)
Contract: 9-month assignment; may extend up to 18 months or convert to temp-to-hire
Schedule: Monday–Friday, 8:00 AM to 5:00 PM
Education Requirement: High School Diploma
Experience Requirement: Minimum 1 year of related experience
Location Requirement: Must reside in one of the following states: AZ, FL, GA, ID, IA, KY, MI, NE, NM, NY (outside Greater NYC), OH, TX, UT, WA (outside Greater Seattle), WI
Equipment Required: Dual monitors and a docking station
Job Summary
The Medical Records Collector supports Molina’s HEDIS and Quality Improvement initiatives by coordinating the pursuit, collection, and uploading of medical records from provider offices. This includes outreach via phone, fax, mail, EMR system retrieval, and onsite collection when needed. The role plays a key part in preparing for annual HEDIS audits and related projects.
Primary Responsibilities

Conduct outreach to provider offices to request, track, and obtain medical records needed for HEDIS projects.

Coordinate record pursuit and collection in collaboration with national and regional HEDIS teams.

Support project management activities related to record identification, retrieval, and documentation.

Assist leadership with preparation and execution of the medical record collection workflow, including phone, fax, mail, EMR retrieval, and onsite pickup.

Participate in meetings with internal teams and external vendors regarding the chart collection process.

Receive, review, scrub, rename, and upload medical records into the HEDIS database.

Research provider and member information using internal and external systems.

Collaborate with team members to meet deadlines, reporting standards, and audit timelines.

Perform onsite chart collection as required.
Required Experience and Skills

At least 1 year of customer service experience.

Basic proficiency with Microsoft Office.

Ability to use Excel for data review and tracking.

Strong organizational skills, including file management and schedule coordination.

Experience with chart retrieval, provider communication, faxing, reviewing, renaming, and uploading records.
Preferred Experience
-
One season or one year of experience collecting medical records.

One year of managed care experience.

Basic understanding of HEDIS and NCQA.
Licenses and Certifications

Valid driver’s license with a good driving record.

Reliable transportation with availability to drive locally for onsite medical record collection.
