Health Education Coordinator (Outbound Calls – Remote)

Location: 100% Remote
Schedule: Monday–Friday, 8:00 AM – 5:00 PM (1-hour lunch)
Employment Type: 4-month Contract (No current FTE conversion potential)
Pay Rate: $16.00/hr




Job Summary

The Health Education Coordinator is responsible for developing, implementing, and evaluating health education programs for members within the network and community. This role also involves making outbound calls to schedule members for annual well visits, assisting providers in meeting contract requirements, and supporting overall population health initiatives. The position functions in a lead capacity, providing oversight to health educators and assisting with administrative decision-making as delegated.




Key Responsibilities

  • Make outbound calls to schedule members for annual wellness visits.

  • Act as a resource to health educators to resolve issues related to intervention protocols.

  • Design, implement, and evaluate health education interventions based on departmental priorities and contractual needs.

  • Conduct population-specific and member-specific needs assessments as required.

  • Lead the development of internal resources in collaboration with other departments.

  • Identify and evaluate health education services within the community and assist with referral agreements.

  • Serve as a liaison to internal departments, community-based organizations, and public health agencies.

  • Provide input on the design and functionality of departmental databases.

  • Coordinate health education services assessment of IPAs and medical groups; collaborate with Quality Improvement on oversight.

  • Prepare and submit required monthly reports and perform other related duties as assigned.




Qualifications

Education:

  • Master’s degree required, or equivalent experience/certification in patient education (e.g., CDE).

Experience:

  • 2+ years of experience with outbound calling and self-motivated outreach work.

  • 0–2 years of experience working with culturally diverse and low-income populations within a managed care setting.

  • Supervisory or lead experience required.

Certifications:

  • None required.




Knowledge, Skills, and Abilities

  • Excellent verbal and written communication skills.

  • Ability to maintain confidentiality and adhere to HIPAA regulations.

  • Strong self-motivation and ability to perform outbound member calls independently.

  • Ability to build positive and effective working relationships with members, providers, and internal teams.

  • Ability to design and execute community-based health education programs.

  • Regular attendance and adherence to company policies and procedures.




Technical Requirements

  • Must have a computer and monitor, with capability to make outbound calls via computer (e.g., headset, softphone, or VOIP system).

  • Stable internet connection suitable for remote work.
